The SF-12 is one of the most common and trusted assessments of physical and mental health used in patient outcomes research. This brief questionnaire contains both a physical health summary score and a mental health summary score. The physical health summary score consists of patient-reported pain, physical function, limitations in daily activities due to physical health, and overall perception of physical health. The mental health summary score consists of patient-reported vitality (energy/fatigue levels), limitations in activities due to mental health, interference with social activities due to mental health, and overall perception of mental health.Â
Over 600 KanodiaMD patients completed the SF-12 questionnaire to track their physical and mental health outcomes as a patient in the practice. Statistical analysis of the data after one-follow up visit and two-follow up visits was performed by Dr. Chris D’Adamo, an epidemiologist at the University of Maryland School of Medicine.
